Unresponsive pupile and more means poor outcome

When there is no pupil reaction (non-reactive pupils) in a patient with unresponsive wakefulness (formerly called vegetative state) following hypoxic brain injury, it typically suggests severe brain damage, particularly involving:

🔬 Prognostic Value of this reflex:

• Fixed, dilated pupils after hypoxic coma = strong predictor of poor outcome.

• Supported by EEG (electrocerephalography), SSEP (somatosensory evoked potentials), and imaging (MRI, CT) to assess the extent of brain damage.

Look at this miraculous response of upper limbs once I commenced Brainstem-Stimulator.

When I applied Cranial Electrotherapy Stimulation and observed upper limb flexion movements in a patient with severe hypoxic brain injury, the response suggests transient activation or modulation of preserved neural circuits. Here’s what is likely happening neurophysiologically:

• This response suggests preserved subcortical motor function.

• Though not necessarily indicative of conscious awareness, it provides a positive biomarker for possible neuroplasticity.

 

Pupil Reaction returns

Right Pupil first time miraculously constrict in 3 minutes.

The observation that at 3 minutes of electrical Brainstem-Stimulation, the right pupil constricted (miosis) for the first time, while the left pupil remained non-reactive, is highly significant. This indicates partial functional recovery or preserved integrity in specific brainstem and diencephalic circuits, especially on the right side.

 

 

 

Started with Neuromodulation via tDCS to the cortex

Applied HDC-STIM for stroke neuro rehabilitation while CES still working together.

And within 5 seconds you observed a pupil constriction and upper limb response.

This is a highly specific neurophysiological interaction and indicates successful coactivation of prefrontal-cortical and brainstem-autonomic circuits.

 

this is likely a primitive reflex release (like decorticate flexor posturing) or partial activation of motor engrams.

🎯 Clinical Interpretation

This stimulation likely co-activated motor, brainstem, and autonomic–affective reflex networks, triggering a highly organized primitive motor pattern that is not voluntary, but suggests preserved integrative motor circuits.

Such responses indicate:

• Functional connectivity between cortex and brainstem still exists

to get more response we try to stimulate conscious cortex

After I prayed a well known Jesus Prayer, put divine fragrance cross mark on the forehead , nose, mental cheek bone hands and feet to stimulate olfactory mammillary limbic networks, plus tDCS and CES she obeys mothers commands and opens mouth when asked to .
